Individual Certification?
Who is considered an individual provider?
An individual provider is a provider who is certifying to completing the training only on behalf of themselves, or is a staff member who is certifying on behalf of only one provider in the practice. The provider or staff member should fill out and submit an “individual certification”. A valid National Provider Identifier (NPI) is required to be entered on the certification form for the provider to receive credit for completing the training.
Group Certification ?
Who is considered a group?
A group is more than one provider who share a valid Tax Identification Number (TIN). If you are a provider or a staff member who is certifying on behalf of a group, fill out and submit a “group certification”. A valid group TIN and a valid Center ID (indicated on your outreach letter) is required to be entered on the certification form for the providers in the group to receive credit for completing the training.
